Member Application

Please use this form to create a free professional account to the FHS website.  After creating your account, you can choose to become a paid member.  A paid membership is available to licensed individuals as well as business owners and support staff that have an interest in elevating patient care and best practices.

After submission, please select your membership type:

Select Non-Paying Member if you do not want access to FHS membership benefits and continuing education.
Select Yearly or Monthly Membership if your are an individual that needs CEUs for your licensing.
Select Associate Member if you are an individual that does not need CEUs and you are an owner or support staff for another member.
